Support tickets cannot be accepted without this completed form. This document will form part of your warranty and answers may be used as proof against any claims.

Once this form is completed and your support ticket is submitted, you will be supplied with a service call log number/SCL number/Ticket number which should be used in any future correspondence regarding the issue.
